:
Introduction: Urinary Tract Infections (UTIs) are common diseases and are treated by antibiotics, and over the years, multiple drug resistance from gram-negative bacteria has emerged. These bacteria produce extended-spectrum beta-lactamases (ESBLs) that make them resistant to antimicrobials. However, the effectiveness of fosfomycin and mecillinam against the ESBL producing bacteria has been shown. This study, therefore, sought to assess the interaction of these antimicrobials with the ESBL producing pathogens from urine samples. Methodology: 25 samples were obtained from the clinical microbiology laboratory at King Abdulaziz University Hospital, and the bacteria were isolated and cultured. Also, the sensitivity of the bacteria to fosfomycin and mecillinam was established using the minimum inhibitory concentration, and the association, invasion, and survival assays were conducted. The cytotoxicity, apoptotic and necrotic effects of the antimicrobials to the urothelial and bacterial cell lines was carried out. Results: The results showed that there were more females than males presenting with ESBL infections. Besides, the effectiveness of mecillinam and fosfomycin antibiotics on ESBL infections was dose-dependent. The E. coli cells had higher survival rates than ESBL E. coli cells in the presence of fosfomycin and mecillinam antibiotics. Further, the ESBL+ urothelial cell associations were lower than the E. coli+ urothelial cells. Further, ESBL producing bacterial strains had higher levels of cytotoxicity than non-ESBL bacterial strains, and mecillinam induced cell death more than fosfomycin. Conclusion: The effectiveness of mecillinam and fosfomycin on ESBL infections was dose-dependent. However, there is a need for more studies that utilize more urine samples.
