Abstract : The present study aimed to determine the association between Single nucleotide polymorphisms (SNPs) of the adiponectin gene (ADPN gene; +45T>G and +276G>T) with circulating total and high molecular weight (HMW) ADPN, obesity(OB) and hypertension(HTN) risk factors in Saudi women. The two SNPs and haplotypes were genotyped in 124 volunteers that were divided into the following groups:G1:control(n=43),G2:obese(n=31), G3:hypertension (n=15), G4:hypertension and obese(n=35). Total and HMW ADPN concentrations, anthropometric and biochemical measurement were determined. SNPs were determined by polymerase chain reaction -restriction fragment length polymorphism (PCR-RFLP) protocol. The concentrations of fasting serum HMW ADPN showed significant differences among the four groups (P<0.05). The correlation between body mass index (BMI)(r = -0.29, P=0.001), waist to hip ratio(WHR)(r=-0.37,P=0.0001), triglycerides(r=-0.25, P=0.005), homeostasis model assessment for insulin resistance(HOMA-IR)(r=-0.35, P=0.0001) and high density lipoproteins (HDL) (r=0.33,P=0.0001) with HMW were observed in all subject studies. In single SNP +45 analysis, the carriers of G/G had significant higher systolic and diastolic blood pressure in G4, and all subject studies in addition to fasting insulin levels and HOMA IR. At SNP+276, higher serum total ADPN in G/G subjects than T/T or T/G (G/G:9.01±0.67 vs. T/T:8.97±1.51 or T/G:7.99±0.93; respectively) for all subject studies. There was no effect of SNP+276 genotypes on anthropometric and biochemical measurement that also not shown in SNP+45/+276 haplotypes in addition to ADPN concentrations . Moreover, The risk of OB was higher in carriers of T allele at SNP+276, whereas the risk of HTN was higher in OB carriers GG at SNP+45 or TT at SNP+276. Regarding the haplotypes analysis, higher risk OB at TG/X haplotype was shown, and the risk of HTN was not shown in lean subjects but was shown in OB carriers X/X haplotype subjects. In conclusion, the present study revealed that HMW ADPN concentration decreased in HTN and OB women, and showed that SNP+45 in ADPN gene has an effect on blood pressure and insulin level. The recorded data suggest that SNP +45T>G and +276G>T genotypes might be involved in an increased risk of OB and HTN
